I would change:

Question # 10

Please separate North America/Central America in two options. Why? Because 
in Central America IT situation is a lot different of Mexico, USA and Canada

I would add a question about which other systems are used in their 
organizations and the percentage of them (This will help us to know about 
the weight that BSD systems have in their IT infrastructure.)

I also would add the a question about the importance of the BSD system in 
their organizations:
Has it a "critical mission server" role?
