FWIW, I sort-of figured this one out.  I ended up replacing the UW
IMAP server with Dovecot.  The performance looks pretty good so far.
Its just annoying how many Dovecot settings had to be altered to
behave like UW.  :(  But a bit of shell scripting managed to convert
MBX formated files into mbox files, merge /var/mail/user with
~user/mbox, and copy ~user/.mailboxlist into ~user/.subscriptions.
Then a global procmailrc file to change new message delivery from
/var/mail/user to ~user/mbox.

So far so good.  I had a glitch with UIDs being too low, but a quick
config change in dovecot.conf and a restart later it was fixed.  :)
