It can, but `pkg upgrade` performs the `pkg update` step for the user
(in case the user forgot).

I haven't forgotten about adding the documentation. Meltdown and
Spectre are taking the majority of my time right now and I can address
the documentation you requested either late tonight or tomorrow.
